The Prognostic Value of Type ¥³Collagen in Squamous Cell Carcinoma of Head and Neck


Abstract
The basement membrane (BM) is an extracellular scaffold found at the epithelium-connective tissue junction and has been considered to the first barrier to by crossed for malignant epithelial cell in the invading process. For this reason, the
organization of BM and the presence of extracellular matrix protein are important factors in influencing the metastatc spread of solid tumors. We determined the influence of lymphocyte infiltration and expression of type ¥³ collagen on prognosis
of
squamous cell carcinoma (SCC) of head and neck..
The clinical significance of type ¥³ collagen, the major basement membrance (MB) component, was examined by immunohistochemistry, using monoclonal anti-type ¥³ collagen antibody in the 45 cases SCC of head and neck. According to the
immunoreactivity,
all specimens could be divided into three groups : Group A, a continuous linear pattern of BM ; Group B, an alteration of BM associated with lymphocyte infiltration ; and Group C, an alteration of BM without without lymphocyte infiltration.
@ES The results were as follows :
@EN 1) In the relationship between T category and immunostaining groups, of 12 cases classified as group A, three cases were T1 9 cases were T2, but there were no cases of T3 and T4 (P<0.001). Of 15 cases classified as group B, 2 cases were T1, 5
cases
T2, four cases T3 and four cases T4. Of 18 cases classified as group C, two cases were T1, 9 cases T2, three cases T3 T3 and four cases T4.
2) In the relationship between cellular differentiation and immunostaining groups, of 16 cases of well differentiate tumors, 8 cases (50%) were group A but of 8 cases of poorly differentiated tumors, 5 cases (62.5%0 were group C.
3) In the relationship between regional lymph node metastasis and immunostaining groups, the metastatic rate for group A was 0%, which was significantly different from group B and C, which were 33.3% and 61.1% (P<0.05).
According to above results, expression of type ¥³ collagen of BM was correlated with clinical manifestations, such as tumor size, cellular differentiation and regional lymph node metastatic rate, therefore the visualization of the staining for
type
¥³
collagen of BM and lymphocyte infiltration in SCC of head and neck appeared not only to increases our understanding of the biologic and clinical behavior tumors, but could be a prognostic indicator.
